There is still hope... Minutes ago wall post in Nissan Leaf Facebook group:

Those Reservationists who were eligible to fill out a survey for a free charger during the Nissan LEAF sign up process as part of the US Government sponsored "EV Project" should be receiving a message today from their partner, ECOtality, via a Nissan email. This communication will detail the results of their evaluation of your eligibility for the unit. If you were offered and have taken the survey, but have not received this email by tomorrow, please call 1-866 NO GAS EV (664-2738) or chat online on the LEAF website.

I am on the needs-further-evaluation list - have a 100A service panel and no open slots.
I don't think it is about the panel. I think it has to do with some of the other questions. Or, maybe, it is due to glitches in the original on-line form entry. I encountered some problems, so it may have submitted "incomplete".

I have a separate dedicated panel with EV-TOU meter running to hardwired junction box in the garage where my old charger was. That certainly isn't what made my situation questionable.

But I said I don't have a smartphone, since I want to know what one will work best with their system before I invest in that. It could be one of those simple answers that is holding up the works.

Got the 'ECOtality has some additional questions' email today too.

Considerations that I think all play a role in this:
a) I live in a old house (1929) but I have a newer 200 AMP panel. Extra slots are easy for the panel although I have to tinker a bit because I have set up the box for convenience and not for load. I have the garage separate from my shop separate from my media room.. you get it. but I don't pull anything close to 20 amps on those so some consolidation will make for a quick fix. (I answered the survey to let them know I had room for the 220v circuit).
2) I have a detached garage (I need to run a 220 line to it from the panel)
3) All the commute questions - remember they are looking for users of the car that will use it frequently and potentially charge at work as well.
4) your location also plays a role. They want drivers in different locations throughout the area.

Hopefully, the issue with mine is just the detached garage and needing to get a 220 line out there from the box (something I plan to do myself).

It is possible that they need to assess for anyone where the install is not a "standard install".

In my case the house is new, panel is in the garage. So, it is a std install and won't cost more than 2200. If it may cost more, they will assess, give an estimate. The owner would pay the difference between 2,200 and the full installed cost.
