Georgia tax credit: lost the form

A year ago I leased my Leaf, submitted the paperwork to the EPD and received the approved form back from them. I put it in what should have been a safe place, but now it's not there! Has anyone else been through this? Do I have to start over, or is there a better way? I plan to e-file, as as I understand it the process is claim the credit and wait for the Georgia IRS to send a letter requesting the form, so I guess I have some time.

I called the EPD today. They can mail a copy to me.
