Help - Federal odometer statement on lease buyout through bank.

I’m looking for advice on working with my credit union and Nissan on a buyout of my lease.

Specifically I’m looking for help understanding the odometer statement I need to provide.

Nissan needs two things, 1. Check from the bank for the buyout amount which can not have my name on it anywhere and 2. A federal odometer statement.

My credit union is working with me on this but they don’t know the details about the odometer statement. I picked up the blank statement document from my DMV. I’m just curious if all I need to complete is the “registered owner” section and then have my credit union send it in with payment.

In other words, it's filled out based on honesty/personal integrity. Similar to sales tax forms on personal used car sales.
