4. Sometimes, definitely for the LEAF.
5. I think it's much simpler than this:
a. For most people to "get" the LEAF, they have to ride in and drive it. Most will not understand until they do.
b. Many people will start thinking seriously about the LEAF when they see friends and neighbors driving LEAFs.
c. Ads and TV/movie placements will help. People can more easily imagine themselves driving a LEAF if they see their favorite actor driving one.

a. at home? (yes/no) Yes, standard outlet though, not 240V.
b. at work (yes/no) No, but nearby if needed, although only needed once when there was a major power outage at my home.
c. other local public access (yes/no) Yes, use when convenient, but never really needed.

4. do you consider yourself an early adopter? Yes, #567 in the USA.

Don't see any of these going to change a conservative. Conservatives never buy anything until it is tested and proven. I would like C, but have plans for mine in mind anyway.

As long as the car can make it to the store and back it will be useful to me. If it has 50% reduced range in 15 years time I will still likely use it and buy a whole new car versus replacement batteries.

6. My battery pack will have reached the end of it's usable life when I can't use it any more. This is not a simple capacity question but other factors will be considered such as the availability AND affordability of DC Quick Charge stations. Well placed DC Quick Charge stations could allow my battery pack to be usable even after it has lost much of its capacity. However, if the stations are too expensive to use they do me no good.
