Rear door lock actuator detached?

I discovered that one of my rear doors will no longer lock correctly.

I checked the child lock setting on the side of the door and confirmed that it's in the "unlock" position. I flipped it back & forth to make sure that it wasn't somewhere in-between.

The mechanical door lock switch works. If I push it to the locked position, the door is locked. If I push it to the unlock position, the door is unlocked.

The electric door lock actuator is what's not functioning. It doesn't change the lock/unlock state of that one door. My son set off the car alarm yesterday by opening that door when the rest of the car was locked.

Has this happened to anybody else? If so, what was the problem & solution?

I plan on tearing the door apart and taking a look at it this weekend. I'm hoping that something just became detached, but dreading that the actuator may have gone bad.
