Updated EV Protocol Document

I took LEAFfan's version and rework it some. It is just the first page now. I changed to the font to Sabon. I added a form element to the PDF version for you phone number. Looks good to me... and a helluva lot better than AB475's mandate that unplugged vehicles get towed.
