california HOV form filling help

I need a quick help from those that have filled out the DMV form 1000 for applying for HOV de-cals in California.

Our car is a leased car, and so the registration card shows both the lessor and the lessee names. The form 1000 asks for owner and co-owner. What should I fill up there? Only the lessee's name (our name), or both. If both, I need to get a signature from the Nissan dealership.
Who is the main "owner" and who is the "Co-owner" and only one signature is required at the 2nd page etc etc.
Appreciate any help with this.

Just fill in your name and address as it appears on the registration. The lessor does not need to be involved.

Just remember to include the registration fee with the application. Otherwise the back and forth mailings will add a month before you receive the sticker.
